RECALL of 603,730 Alaris 8100 infusion pumps-Major Material Flaw

In another recall of the Alaris 8100 infusion pump, Becton Dickinson issued a Class I recall for 603,730 potentially life threatening manufacturing defect. A plastic part holding the defective tubing (mentioned below) in  place can and has cracked allowing  Freeflow and over infusion of medication in patients.

On July 18, 2019, 151 Million Alaris 8100 infusion sets were RECALLED by Becton Dickinson due to a silicon tube defect that caused Overdoses of dangerous medications that resulted in  severe injury and death. (This device had been marketed as Carefusion) 

 Manufacturer wrote to hospitals in its recall: 

"This recall has been associated with MDR reports, several of which are associated with serious injuries."  Medical Device Reports or MDR are made by doctors and hospital to the FDA.

Becton Dickinson manufactured and then distributed around the world 151 million life threatening defective silicon tubes without noticing.  What a quality control disaster!!!  

In this design the pump pinches or occludes the silicone tube to stop the flow of dangerous medication to the patient.  This admitted defect in the tube allowed the medicine to continue to be pumped into the patient even though the electronic monitor told nurses and doctors that no medicine  was being infused, in what is termed a "free flow" that resulted in an over infusion. This outdated pump DOES NOT measure, either mechanically or electronically, the actual amount of medicine pumped into the patient!!!! The pump assumes delivery of the programed amount by a mathematical calculation therefore nurses and doctors have no way of knowing what went into their patient because the pump will report everything had been delivered correctly.

FDA Protects Medical Device Manufacturers

The U.S. Food and Drug Administration issued a Statement March 15, 2019, where it begrudgingly acknowledged it failed to protect Americans for serious injury and even death.    https://www.fda.gov/NewsEvents/Newsroom/PressAnnouncements/ucm633602.htm​
Apparently, the FDA felt it had to respond to the points made in the excellent Netfix documentary "The Bleeding Edge" by Writer/Director Kirby Dick and Amy Ziering. If you haven't seen it please take the time and make sure to bring a box of tissues. These wonderful filmmakers and the brave subjects bring to light the power of the Medical Device Industry (worth $40 Billion with a B) over our the FDA.  The subjects of the film from the orthopedic surgeon poisoned by metal from his own hip replacement to the thousands of women suffering unrelenting never-ending pain from the metal wire contraceptive device Essure, illuminate that the FDA miserably failed to protect innocent patients from manifest threats to body and soul of people throughout this country and the world.

On March 8, 2019, Kaiser Health News exposed a secret mountain of reports of malfunctioning medical devices regulated by the FDA which were virtually hidden from the public, doctors, and patients. https://khn.org/news/hidden-fda-database-medical-device-injuries-malfunctions/   "Since 2016, at least 1.1 million incidents have flowed into the internal “alternative summary reporting” repository, instead of being described individually in the widely scrutinized public database known as MAUDE, which medical experts trust to identify problems that could put patients in jeopardy" according to Christina Jewett of Kasier Health News.  In particular, Ms. Jewett found at least one device, with life threatening implications, a surgical stapler had 10,000 reported malfunctions which were not found in the relied upon data base.  In fact, only  100 such malfunctions where found during that time.
