In another recall of the Alaris 8100 infusion pump, Becton Dickinson issued a Class I recall for 603,730 potentially life threatening manufacturing defect. A plastic part holding the defective tubing (mentioned below) in place can and has cracked allowing Freeflow and over infusion of medication in patients.

In particular, Ms. Jewett found at least one device, with life threatening implications, a surgical stapler had 10,000 reported malfunctions which were not found in the relied upon data base. In fact, only 100 such malfunctions where found during that time.
